Home Care Plans

As each Client has different needs they receive an individual care plan.This plan is drawn up to include the persons next of kin,Doctor,details of illnesses or disabilities,medications taken,diet requirements and details of personal care required.The care plan also contains the duties that the care giver is required to carry out. On completion of duties the care giver will fill a daily activities form,this will include how the client is both physically and mentality.If any change is detected they report this to the care manager immediately.
